To order a Levitra sample:
- download the 'Levitra sample request form'
- complete ALL of the sections
- fax the completed form to 0113 201 8100
Note: Healthcare professionals are only permitted to receive 10 samples in a 12 month period. If your request will cause you to exceed this level, Bayer Schering Pharma will send you enough samples to take you to the maximum of 10.
